📌 Submission Requirements

  • Staff full name and phone number (required for dashboard login)
  • Ward/Department information
  • Training title and date of completion
  • Training category (Category 1 or Category 2)
  • Number of CEU credits earned
  • Certificate of completion (PDF/Image upload)
  • Training provider/organization

  • Step 1: Ward focal person submits staff credits via this form
  • Step 2: CPD head reviews and verifies the submission
  • Step 3: CPD head adds verified credits to Google Sheet
  • Step 4: Staff can view their credits using phone number on CEU Dashboard
  • Processing time: 2-5 business days
