  • Staff full name and phone number (required for dashboard login)
  • Ward / Department information
  • Training title, date, and category (Category 1 or 2)
  • Number of CEU credits earned
  • Certificate of completion (PDF / Image upload)

Submission Workflow

1️⃣

Step 1 – Submit

Ward focal person submits staff credits via this form with supporting documents.

2️⃣

Step 2 – Review

CPD head reviews, verifies, and adds approved credits to the Google Sheet.

3️⃣

Step 3 – View Credits

Staff can view their verified credits on the CEU Dashboard using their phone number.
